Equipe 1 : « Adaptations Nutritionnelles, Environnement et Diabètes »

La réponse appropriée aux modifications de notre environnement est essentielle au maintien de l’homéostasie. A tous les stades de la vie, depuis l’embryon jusqu’à un très grand âge, nous sommes soumis à des changements, transitoires ou chroniques, de notre mode de vie, de notre alimentation et exposé à de nombreuses substances toxiques présentes dans notre environnement. La capacité de s’adapter à ces changements conditionne en grande partie la santé et la durée de vie. Notre hypothèse est que des anomalies dans les mécanismes d’adaptation, en ne permettant pas une réponse appropriée à l’environnement, contribueraient au développement de l’obésité, du diabète de type 2 et de leurs complications.

Les principaux objectifs de l’équipe 1 de laboratoire CarMeN sont de comprendre les mécanismes impliqués dans la réponse physiologique aux stress environnementaux chez l’homme, afin d’identifier des défauts potentiels dans ces processus chez les patients diabétiques et chez les sujets obèses, et de développer de nouvelles stratégies de lutte contre ces maladies métaboliques et nutritionnelles.

Tous nos projets combinent des approches complémentaires de recherche clinique, des études sur modèles animaux et une recherche expérimentale in vitro. Cette recherche transversale est possible grâce à l’association efficace de chercheurs et enseignant-chercheurs spécialistes en biochimie, en biologie cellulaire et moléculaire, en immunologie et en physiologie avec des cliniciens de la diabétologie et de la nutrition, du métabolisme  et de la néphrologie.

L’équipe 1 est principalement localisée dans les locaux de nouveau bâtiment Cens-Eli de l’Hôpital Lyon Sud (Bâtiment 2D. Secteur 2) (plan d’accès), et le service de biochime de l’Hôpital Lyon-Sud, à proximité immédiate du Centre de Recherche en Nutrition Humaine – Rhône-Alpes . Une partie de l’équipe travaille dans les locaux de l’IMBL (plan d’accès) sur la campus de la Doua à INSA de Lyon.

Thématique générale de l’Equipe 1

Les changements de notre environnement nutritionnel sont en partie responsables de « l’épidémie » actuelle de surpoids, d’obésité et de diabète.

Les objectifs principaux de nos projets de recherche sont 1) de comprendre, les mécanismes moléculaires de la réponse et de l’adaptation aux changements de l’environnement nutritionnel et 2) d’identifier quelles sont les anomalies dans ces processus qui pourraient conduire aux maladies de la nutrition. Nous avons choisi d’aborder ces questions à partir de situations expérimentales susceptibles de représenter les conditions de vie réelle impliquées dans le développement des maladies nutritionnelles et métaboliques. Sur la base des avancées significatives réalisées au cours des dernières années, notre stratégie repose sur 5 programmes complémentaires de recherche:

1) « Adaptation métabolique des tissu adipeux au cours de la suralimention » pour comprendre le remodelage des dépôts de tissu adipeux pendant la prise de poids et pour proposer des stratégies nouvelles pour lutter contre l’obésité et prévenir ses complications

2 ) « Cellules souches adipocytaires dans les tissus adipeux sains et pathologiques » pour comprendre les mécanismes à l’origine des altérations fonctionnelles et de l’inflammation du tissu adipeux qui sont initiatrices des complications de  l’obésité

3)  » Impact des polluants environnementaux dans les maladies métaboliques » pour démontrer la contribution et les mécanismes d’action des polluants dans l’initiation ou l’amplification des maladies métaboliques

4) « Adaptation métabolique dans la maladie rénale chronique » pour définir les déterminants des complications métaboliques, en particulier la résistance à l’insuline, dans la maladie rénale chronique et proposer de nouveaux traitements;

5) « Les probiotiques pour lutter contre les maladies métaboliques »  afin de démontrer que des souches bactériennes sélectionnées peuvent être de puissants agents pour traiter les patients obèses et diabétiques

Principales méthodologies et outils

Axe 1 : Adaptation métabolique des tissus adipeux au cours de la suralimentation

Un apport calorique excessif par rapport aux dépenses énergétiques est une des caractéristiques du mode de vie moderne, contequipe1_fig3ribuant de façon majeure au développement de l’obésité. Les mécanismes qui régulent les réorientations métaboliques chez l’homme lors de la surnutrition (stockage ou catabolisme des calories excédentaires, régulation de l’expansion du tissu adipeux, stockage ectopique des graisses) sont très mal connus. Ils sont en général abordés par des études de perte de poids, qui ne représentent pas la réalité du développement de l’obésité. En collaboration avec le CRNH-RA, nous développons un programme de recherche ambitieux pour caractériser ces mécanismes en utilisant des protocoles de suralimentation chez l’homme et chez l’animal.

Dans les études cliniques, des volontaires sont soumis à différents protocoles de suralimentation au cours desquels ils consomment environ 30% d’excédent calorique chaque jour pendant plusieurs semaines. Une analyse détaillée des modifications biologiques, métaboliques et anthropométriques est effectuée tout au long des protocoles. Nous testons différents types de suppléments caloriques (lipides, fructose ou « junk food »), mais également de nouvelles stratégies pour prévenir les effets délétères de la suralimentation (polyphénols, probiotiques). Des résultats importants ont déjà été obtenus ces dernières années, nous permettant de comprendre les mécanismes moléculaires de la répartition des substrats énergétiques dans le muscle squelettique ainsi que les premières étapes du développement du tissu adipeux quand les sujets prennent du poids (Seyssel et al, JCEM, 2014; Alligier et al JCEM, 2012).

De façon intéressante, certains sujets présentent une orientation préférentielle du stockage des lipides dans le tissu adipeux viscéral et d’autres dépôts ectopiques tels que le foie et le muscle. Cela suggère que ces sujets présentent probablement un risque plus élevé pour le syndrome métabolique. Nous étudions actuellement plus en détail ces sujets avec l’objectif d’identifier de nouveaux biomarqueurs de risque en utilisant des approches transcriptomiques du tissu adipeux et du muscle squelettique et une approche métabolomique dans le sang et l’urine. Parallèlement aux études cliniques, les mécanismes de l’accumulation de graisse et les fonctions des adipocytes sont étudiés in vitro à l’aide d’adipocytes humains en culture primaire.

Axe 2 : Cellules souches adipocytaires dans les tissus adipeux sain et pathologique

Les cellules souches mésenchymateuses, présentes en grand nombre dans le tissu adipeux (Adipose Stem Cells, ASC), ont non seulement la capacité de se différencier en adipocytes, mais possèdent aussi de nombreuses fonctions régulatrices notamment vis à vis des cellules immunes. Nous explorons la contribution de ces cellules au fonctionnement du tissu adipeux sain et à son évolution pathologique au travers de plusieurs axes de recherche.

Un de ces axes de recherche (A. El Jaafari et L. Pirola) vise à mieux comprendre le rôle de  l’inflammation dans la pathogénicité du tissu adipeux. Alors que la présence des macrophages dans le tissu adipeux a été montrée comme contribuant à l’inflammation de bas grade associée à l’obésité, nous avons récemment identifié 2 nouveaux acteurs dans ce processus: les cellules souches mésenchymateuses dérivées de tissu adipeux (ASC) et les lymphocytes Th17 (El Jaafari et al, Diabetes, 2015). Nous avons démontré que les ASC préparées à partir du tissu adipeux des personnes obèses (mais pas à partir de donneurs sains) peuvent augmenter la sécrétion d’IL-17A et d’IL-1beta par les cellules mononuclées du sang. Ces cytokines inhibent en retour l’adipogénèse et diminue la réponse à l’insuline des adipocytes. Nous cherchons maintenant à identifier les mécanismes en cause, en particulier les molécules impliquées dans l’interaction cellulaire entre ASC et lymphocytes Th17, qui pourraient représenter des cibles nouvelles pour combattre l’inflammation dans l’obésité.

Un autre projet de recherche, développé par A. Mey, vise à comprendre l’origine des différences fonctionnelles entre les dépôts de tissu adipeux et leurs contributions dans l’obésité. Elle s’intéresse pour cela aux spécificités moléculaires et biologiques des cellules souches mésenchymateuses (ASC) qui sont à l’origine développementale de ces dépôts et qui restent présentes dans les dépôts à l’âge adulte. Afin de comprendre comment l’environnement nutritionnel impacte les propriétés des ASC aux différents âges de la vie nous avons développé l’isolement et la culture de ces cellules à partir de différents dépôts de tissu adipeux chez la souris et l’homme.

Une des propriétés intéressantes des ASC est leur possibilité, en tant que cellules souches, de se différencier en différents types cellulaires. Nous avons mis au point un protocole pour générer des cellules bêta-pancréatiques à partir des ASC humaines. Nous allons maintenant utiliser ces cellules pour étudier l’impact de l’inflammation médiée par les cellules souches sur les cellules bêta afin de mieux comprendre les mécanismes conduisant à leur dysfonctionnement dans le diabète de type 2. Nous espérons également optimiser la production de cellules bêta à partir d’ASC pour proposer à l’avenir ces cellules comme outils thérapeutiques. Ceci sera réalisé par A. El Jaafari, en collaboration avec les membres de l’équipe 3 (A-M Madec, C. Thivolet) et de l’Unité de thérapie cellulaire des hôpitaux de Lyon, qui ont récemment rejoint l’équipe.

 

Axe 3 : Impact des polluants dans les maladies métaboliques

En plus de la génétique, de l’excès de calories et du manque d’activité physique, il est maintenant clairement démontré que les polluants environnementaux, en particulier ceux qui possèdent des activités de perturbateurs endocriniens et qui agissent à très faible dose, contribuent à l’épidémie de maladies métaboliques. Plusieurs études épidémiologiques ont montré une corrélation significative entre les critères du syndrome métabolique et la présence dans le sang ou dans l’urine de polluants ou de leurs métabolites. En outre, des études expérimentales chez la souris, y compris les travaux de notre équipe (Naville et al, FASEB J 2013; PlosOne, 2015; Ruzzin et al, EHP 2011) ont établi un lien de causalité entre l’exposition aux produits chimiques et les dysfonctionnements métaboliques liés à l’obésité, tel que la résistance à l’insuline.

D. Naville et B. Le Magueresse dans l’équipe, ont développé un modèle d’exposition de souris à un cocktail de polluants (dioxine, PCB153, DEHP et BPA) à doses très faibles, et ajoutés à une alimentation enrichies en saccharose et lipides aux mères puis à leur descendance. Ce protocole nous permet d’étudier l’impact d’un mélange de polluants à doses environnementales sur les paramètres biologiques, la masse grasse, la tolérance au glucose et la sensibilité à l’insuline dans la souris et sur sa descendance. Afin de comprendre l’origine moléculaire des altérations métaboliques, une approche moléculaire gène-candidat ou globale de type transcriptomique est développée.

En parallèle, nous explorons le phénotype des souris PXR KO en collaboration avec le groupe de J. Ruzzin, Université de Bergen, Norvège et nous utilisons divers modèles cellulaires in vitro. Enfin, nous avons émis l’hypothèse que des variations de microbiote intestinal pourraient modifier la façon dont les individus métabolisent et répondent aux polluants. Nous allons donc tester si des modifications du microbiote induites par l’utilisation de probiotiques pourraient protéger contre  les effets délétères des polluants (cf. Axe 4).

 

Axe 4 : Adaptation métabolique dans la maladie rénale chronique

L’insuffisance rénale chronique (IRC) est fréquemment associée à une résistance périphérique à l’insuline, qui est un important facteur de risque cardiovasculaire. Cependant, les raisons pour lesquelles les patients insuffisants rénaux développent cette résistance à l’insuline, même en l’absence de surpoids ou d’obésité, sont en grande partie inconnues.

D. Fouque et Ch. Soulage dans l’équipe ont récemment démontré que l’accumulation de toxines urémiques, tels que le p-crésol, pourrait être un facteur clé pour le développement de la résistance à l’insuline dans la maladie rénale au stade terminal (J Amer Soc Nephrol, 2013). En utilisant un modèle expérimental d’ IRC chez la souris, nous tentons actuellement d’étendre ce concept à d’autres toxines urémiques, comme indoxyle sulfate, et essayons de définir leurs mécanismes d’action, en mettant l’accent sur l’activation des récepteurs nucléaires, tels que le récepteur AhR (qui est aussi le récepteurdes dioxines). Les mécanismes sous-jacents au développement de la résistance à l’insuline sont aussi étudiés in vitro en utilisant des adipocytes et des cellules musculaires en culture incubées avec des toxines urémiques. L’identification des voies de signalisation contribuant aux effets délétères de ces composés et le rôle potentiel d’AhR seront étudiés en détail dans ces systèmes in vitro. Nous allons ensuite vérifier la pertinence de ces résultats dans notre cohorte de patients atteints de néphropathie chronique. Le rôle de la flore intestinale sera également étudié dans le but d’élaborer des stratégies pro / prébiotiques pour moduler la production de toxines urémiques.
Un autre projet de recherche porte sur l’étude de l’impact de la dialyse péritonéale sur le développement et les fonctions du tissu adipeux blanc intra-abdominal, en utilisant un modèle original de dialyse péritonéale chez la souris (en cours de développement dans le laboratoire). La pertinence de ce modèle pré-clinique sera évaluée dans les biopsies de tissu adipeux sous-cutané et intra-abdominal prélevées chez des patients traités par dialyse péritonéale. Ces échantillons biologiques permettront d’analyser les effets de la dialyse sur la taille des adipocytes, l’histologie du tissu (infiltration de cellules immunitaires, fibrose), les fonctions des adipocytes et la sécrétion adipokines.

Axe 5 : Utilisation de probiotiques pour lutter contre les maladies métaboliques

L’utilisation des probiotiques dans le but de moduler la glycémie et donc d’améliorer le diabète a été envisagée depuis longtemps, en particulier en tant que supplément nutritionnel, et la découverte du rôle critique de microbiote intestinal dans les maladies métaboliques a fortement stimulé la recherche dans ce domaine. L’obésité et le diabète de type 2 sont en effet associés à une perturbation de la composition et du fonctionnement du microbiote intestinal (appelé dysbiose), caractérisée globalement par le changement dans le rapport entre Firmicutes (Gram-positif) et Bacteroidetes (Gram négatif). En outre, il a été démontré récemment que le microbiote intestinal joue certainement un rôle essentiel dans la pathogénie de ces maladies.

Plusieurs souches bactériennes, utilisées comme probiotiques, ont montré récemment des actions bénéfiques sur la tolérance au glucose et la sensibilité à l’insuline chez les souris. Cependant, les effets sont moins importants chez l’homme et leurs mécanismes d’action ne sont pas encore bien compris. Notre objectif est donc d’identifier des souches bactériennes d’intérêt, possédant des effets bénéfiques forts pour la santé humaine, et de les proposer en tant que nouveaux probiotiques pour la prévention et/ou le traitement des maladies métaboliques.
Pour identifier ces nouvelles souches, nous travaillons en étroite collaboration avec l’équipe de F. Leulier à l’IGFL (un leader dans le domaine des interactions hôte-bactéries chez la drosophile  qui a mis au point une méthode innovante pour qualifier les souches de Lactobacillus avec des propriétés anti-diabétiques (brevet déposé en février 2016).

Nous avons récemment utilisé ensemble une variante de ce système de sélection pour démontrer pour la première fois le rôle critique du microbiote et d’une des souches de L.plantarum sélectionnées dans la croissance juvéniles (Schwarzer et al. Science 2016). Nous sommes actuellement en train de qualifier des  souches pertinentes pour leurs propriétés anti-diabétiques, qui sont tout d’abord tester chez les souris diabétiques, avant leur évaluation chez les patients. En parallèle, comme indiqué dans les axes 2 et 3 ci-dessus, les souches bactériennes avec des propriétés probiotiques sont aussi évalués pour leur potentiel à contrer les effets toxiques d’un cocktail de polluants chez les souris exposées et à contrôler la production de toxines urémiques dans un modèle de souris avec insuffisance rénale chronique.
